Despite proposed rules, questions persist about insurance exchanges

The U.S. Department of Health and Human Services released proposed rules for state health insurance exchanges required the by last year's health care reform law, but questions remain about how the exchanges will be implemented.

The U.S. Department of Health and Human Services released two sets of proposed rules Monday that draw the clearest picture yet of how states will implement a key provision of the federal health reform law. But the new rules for state health insurance exchanges, which are meant to offer low-cost, high-quality health benefits to individuals and small businesses, leave many questions unanswered.
