SAN FRANCISCO - In the largest Medicare fraud settlement in the nursing home industry, Beverly Enterprises Inc., agreed Thursday to pay $175 million to settle federal allegations that it engaged in a nationwide scheme to inflate nursing costs, defrauding Medicare of $460 million since 1992.
